WISH PROCESS

Step 1

Please complete the Wish Referral Form. You may complete the form on the family's behalf as long as the family is aware of the referral. Please contact Wishes & More if you do not receive a confirmation phone call within 3 business days of submitting the wish request.

Step 2

Wishes & More will submit a Medical Eligibility Form to the child’s doctor to verify the child qualifies for a wish. Once Wishes & More receives the approved Medical Eligibility Form back from the doctor someone will contact the family and request additional information by completing the entire Wish Application. The family will complete the Wish Application and return it to Wishes & More by fax or regular mail.

Step 3

Wishes & More will interview the child to find out what the child’s wishes are.

Step 4

Wish requests will be reviewed by the Wishes & More Board of Directors for approval and the doctor will verify the child’s wish is medically appropriate.

Step 5

The wish is granted by Wishes & More and the child receives his/her wish.
