      Wish Referral Form

 

Who can refer a child? 
Wish referrals to Wishes & More can be from the child, the child’s parent or legal guardian, by a medical professional treating the child such as a nurse, doctor, specialist, or social worker.  We request the family be made aware that a referral has been made.

 

What is the process to refer a wish child?

Referring a wish child is easy.  Simply fill out the Wish Referral Form so we may obtain some basic information to begin the process.

 

What are the conditions that qualify a child for a Wishes & More wish? 
A child will qualify for a wish (and more) if they have not reached their 19th birthday and have been diagnosed with a terminal or life threatening illness or condition which will require ongoing treatment. 

 

Does the child have to live in Minnesota? 
No, the child may live in Minnesota or bordering states or be treated in a Minnesota medical facility.

 

When is a good time for the family to apply? 
Anytime - Each circumstance is different.  We are prepared to process wishes on an immediate basis or will work with the family to determine the timeframe most appropriate for their needs.

 

Notable Comments:

 A child cannot have received a prior wish from any other wish granting organization.

 Qualifying illnesses and conditions may include: Cancer, Cystic Fibrosis, heart conditions, life-threatening burns, Lymphoma, major organ diseases, and transplants, to name a few. 

 If a medically eligible child should pass away prior to completing their wish, a memorial donation will be given to the family.
