Qualifying Criteria

Wishes & More grants wishes to children from birth to their nineteenth birthday who have been diagnosed with a life threatening illness.

Wishes & More
serves children who live in Minnesota and the bordering states, including Wisconsin, Iowa, North Dakota, and South Dakota. Wishes & More will also grant wishes to children who do not live in Minnesota but are receiving medical treatment in a Minnesota facility.

Wishes & More is unable to grant a wish to a child who has been granted a wish by another wish granting organization.

 

 


POTENTIAL WISH RECIPIENT
or IMMEDIATE FAMILY MEMBER

A potential wish child or immediate family member should complete the REFERRAL FORM (this is the first page of the full application). 

The Director of Wish Services will send a MEDICAL ELIGIBILITY FORM to the doctor.  The doctor will evaluate the eligibility and return the form to Wishes & More.  If the recipient is eligible, the complete APPLICATION FORM will be sent to the family.  If the recipient is not eligible, a phone call will be made to the family along with a follow up letter sent stating the recipient is not eligible at this time.

When the completed wish APPLICATION FORM has been returned to Wishes & More, the family will be contacted by the Director of Wish Services.

The completed form should be returned to Wishes & More by fax or regular mail.
